The nurse practitioner remains the most responsible provider through end of life, and we coordinate hospice services on the family’s behalf.

Gloria Holder
Nurse Practitioner and Founder
Gloria Holder has practised nursing for more than twenty years. She is the founder of Rooted Care and Living and the most responsible provider for every client: she carries out health assessments, diagnostics, and medication administration, assesses each prospective client to confirm the home can meet their needs, and trains the personal support workers who work alongside her.
